So if I read your issue correctly, the only switch not working is the 3-way (aka accessory) switch? Once you included it into your network did you go through the steps to "associate" it with the device you want to control?
This took me a little bit to get the hang of at first but now that I get it, I actually have a 5 way switch (thank you crazy ass builder for putting switches everywhere...). The basic idea is that the accessory switch doesn't technically control the light, the accessory switch controls the switch that controls the light...and that is done through "association".
So if you click your accessory switch in the app, then click the "associations" button in the lower right corner, it will bring up a list of all your devices. You then check the box of what you want to control with that accessory switch and close. Now that switch is like the Z-Stick controller in that when you press a button on the switch it sends a signal to the target switch to turn on/off etc.
That said, I have found that you can't really control an accessory switch through the UI...but then again, why would you want to...control the loaded switch from the UI.

First off, I think I fixed the issue, but to clarify my setup:
Only one switch out of the three switches that are a part of the 3-way switching scheme is a Z-wave device (the master) - the other two are just companion devices (the slaves) that, as you stated, tell the master how to control the load. In the same 2-gang box is another Z-wave device that is the master for a completely separate 3-way switching scheme. Both of the Z-wave switches pair just fine, both switches show up in the HA software and don't report any errors - seemingly working correctly. Except that only one of the two switches responds to commands through the software.
Ultimately, I ended up resetting my z-stick, going around and excluding all the devices, and then re-including them. Afterwards I had the HA software "autoconfigure" each of the devices and things started working. I'm not sure why going through and resetting the whole thing was required, but it's working pretty well now. In my troubleshooting I ended up swapping the non-responsive master devices with one of it's companion slave devices to help separate the two Z-wave devices being in the same 2-gang box, but I don't think this really had any effect on the end solution.
Just wanted to update the thread in case anyone else runs into a similar issue.
